What Is the NHTD Waiver?
The NHTD Waiver is a Medicaid-funded program designed to support individuals who would otherwise need nursing home care. It provides community-based services that allow eligible adults to live safely and independently in homes of their choosing rather than in institutional facilities.

Who Is Eligible?

The Transition Process: Step-by-Step
1. Get Started with a Referral
If the individual is currently in a nursing facility, the facility’s discharge planner can refer them to their local Regional Resource Development Center (RRDC). Those living in the community can also reach out to the RRDC directly to begin the process.
2. Complete an Assessment
The RRDC will coordinate a review to verify Medicaid eligibility and assess the individual’s need for nursing-level care.
3. Create an Individual Service Plan (ISP)
If found eligible, the person will begin working with a Service Coordinator to build a personalized plan. This ISP outlines the specific supports and services they’ll need to live safely and comfortably in their new setting.
4. Secure Housing and Services
Part of the transition may include help finding housing, setting up home modifications, or securing rental assistance. The ISP will also address any personal care or in-home support services the participant may need.
5. Put the Plan Into Action
Once housing and services are in place, the individual will officially transition to their new home. The Service Coordinator will monitor the plan’s effectiveness and make updates as necessary.
6. Understand the Timeline
In general, the transition process takes about 3 to 6 months. This can vary based on how quickly documents are submitted and how easily services can be arranged.
What Paperwork Is Required?
To move forward with the NHTD application, participants will need to provide:
Proof of Medicaid enrollment
Documentation of age or qualifying disability
Medical records indicating a need for nursing home–level care
Information about a potential living arrangement
Financial records, such as income and assets
Services Available Under the NHTD Program
Each service plan is customized to meet the individual’s specific needs. Depending on the plan, services may include:
Service coordination
Personal care assistance
Independent living skills training
Home modifications and assistive technology
Transportation
Respite care for family caregivers
